Freeze/Cancellation Form

We get it, life happens. Fill out this form to request to freeze or cancel your membership.

Please keep in mind that if you are within your initial term agreement and your request does not fall within the medical/travel/relocation reasons for freezes & cancellations, we may not be able to accommodate your request.

You will receive an e-mail confirmation along with a PDF copy of your F/C request. Member Services will review your request and follow up as soon as possible!

If you find that your reasons for freezing/cancelling are outside of the contract agreement, shoot us an e-mail at memberservices@kravmd.com and we will review.
